Once you book your Initial Assessment HERE! You will receive all the necessary information regarding preparation for your appointment. Prior to your initial assessment, you will need to fill out our intake forms. If you require us to bill or contact your insurance company directly, you will need to let us know in the intake form, prior to your appointment, so that we can send you proper insurance related forms and help you fill them out. Because PhysioD is a boutique-like, private clinic that only accepts one person at a time, we require everyone to be ontime for their appointments.
